That intuitive "feeling", or gut instinct about something, is your mind processing subconsciously things you observe. The subconscious aspects of our personalities tend to speak to us in physical terms while awake, and in dream when asleep. This is my take on it though. The "gut" is a common place for people to feel these conclusions that our more animal and instinctual side to communicate with. Essentially, in the context of someone suspicious of their surroundings, its the "flight or Fight" mechanism activated.
We are taught to ignore those types of signals. Guilt, shame, excitement, many other states can be felt in the body. Learn to listen.
And to answer your question finally, I feel the gut BEFORE the actual circumstance. If particularly strong, I will feel it the entire time I am around someone I mistrust. It can develops into a nagging doubt about someone.
It is upon reflection of "WTF did I just miss?" or "Something doesnt seem right", that I consciously begin to examine the details.
Like a jigsaw puzzle that you don't have the "Big Picture" of, you still have the pieces scattered before you. Once you see enough pieces, your mind begins to form a foggy picture based on those. We intuitively fill in the blanks. The more pieces, the better the picture.
There are certain situations however, I try my damned best to listen to my body and not my head you know?
That all being said, there are times my intuition or instincts are incorrect. Better safe than sorry and Never Get Busted!
